Longview Regional Medical Center
Employee Summary of Benefits
All full-time employees working 32 or more hours per week and all part-time employees working 20-31 hours per week are eligible to enroll in some or all benefits offered by Longview Regional Medical Center. Your benefits will be effective on the first of the month following 30 days of employment.
All benefits must be enrolled in through the online enrollment system accessed by going to www.chsbenefits.net within 30 days of first becoming eligible. The enrollment system will be open
until the last day of the month prior to the benefits effective date.
Medical Plan
- Medical Claims Administrator—The CHS Medical Plan will be administered by MedPartners Administrative Services. You will receive new ID Cards
- Medical Plans—There are two plans offered, Premier and Basic
- Annual Deductible—The Premier Plan Deductible is $615 per Individual ($1230 per family)
The Basic Plan Deductible is $1485 per Individual ($2970 per family)
Prescription Claims Administrator
The Prescription Plan will be administered by CVS Caremark. You will receive separate ID cards for
your prescription coverage.
- Prescription Drug Plan—The Premier and Basic Medical Plans have an annual prescription deductible of $150 per individual ($450 for family)
- The retail co-pay for Generic is $15
- Preferred (formulary) brand drugs is $35
- Non-preferred (non-formulary) brand drugs is $55
- Mail order co-pay for Generic is $30 = 3 month supply
- Mail order for Preferred (formulary) brand drugs $70 = 3 month supply
- Mail order for Non-preferred (non-formulary) brand drugs $120 = 3 month supply
- The Specialty drug co-pay is $80 for a 30 day supply
Dental Plan
Employees who are regularly scheduled to work at least 20 hours each week may enroll themselves and their dependents in the Group Dental Plan administered by Delta Dental. Benefits under this plan are based on usual and customary fees. Any licensed Dental provider may be used; however the Delta
Dental Premier and PPO networks will provide greater savings. Dental benefits are described in detail
on page 11 of “Your Flexible Benefits Enrollment Guide”.
Vision Care Plan
The VSP Vision Care Plan offers benefits for services provided by both network and non-network providers. Employees must work at least 20 hours each week to be eligible for this coverage. The
Vision Benefits are described in detail on page 12 of “Your Flexible Benefits Enrollment Guide”.
Company Paid Life Insurance and AD&D
Employees working a minimum of 32 hours per week are provided basic life insurance and accidental death & dismemberment coverage through Sun Life in amounts of either one or two times their annual salary based on their employment position. This is a company paid benefit and is provided at no cost
to eligible employees.
Supplemental Life
Employees working a minimum of 20 hours per week may purchase additional life insurance for themselves and their dependents through Sun Life. No health history questionnaire (EOI) is required
for this coverage.
Employees who qualify for company paid life insurance may purchase supplemental life in multiples of
1x to 4x increments of their base salary. The combined company paid and supplemental life cannot exceed 5x base salary (up to a max of $1,500,000). Rates for supplemental life are based on age and amount of coverage and are listed on page 16 of “Your Flexible Benefits Enrollment Guide”. The online benefit enrollment system will calculate your rate per pay period for you. Part-Time employees (20-31
hrs per week) may purchase supplemental life insurance in $15,000 increments (up to a max of
$75,000).
All employees working a minimum of 20 hours per week may also purchase $25,000 dependent life for their spouses and $10,000 for dependent children.
Short Term Disability (STD)
Employees working a minimum of 20 hours per week may purchase short-term disability coverage
which provides 60% of base salary (max weekly benefit of $2,000). Benefits for certified disabilities
begin after a 14 day elimination period and may pay up to 11 weeks. Rates are based on age and
amount of coverage and are listed on page 17 of “Your Flexible Benefits Enrollment Guide”.
Long Term Disability (LTD)
Employees working a minimum of 20 hours per week may purchase long-term disability coverage
which provides 60% of base monthly salary (max monthly benefit of $5,000). Benefits for certified disabilities begin after 90 days of continuous disability. Rates are based on age and amount of
coverage and are listed on page 17 of “Your Flexible Benefits Enrollment Guide”.
ID Theft
The ID Theft Assist Program assists with all aspects of restoring and recovering the credit and identity
of a victim of ID Theft. ID Theft Assist helps protect employees and their families from financial liability* and helps to eliminate the work, worry and aggravation identity theft can cause. ID Theft Assist will
offer:
- Proactive, full credit monitoring service, including credit report and score for one household member
- Assistance in replacing personal ID documentation such as driver’s license, passport, and
Social Security card
Employees must work at least 20 hours each week to be eligible for this coverage. Additional
information and rates are listed on page 18 of “Your Flexible Benefits Enrollment Guide”.
*Note: This plan is not an insurance program and does not reimburse for any fraudulent charges or expenses incurred due to an identity breach or theft.
Adoption Assistance Plan
The adoption plan will reimburse up to $2,000 for expenses within the plan year relating to the
adoption of a child. For a “special needs” child the plan will reimburse up to $3,000. The annual max benefit is $4,000 with a max lifetime benefit of $5,000.
401 (k) Retirement Savings Plan
- All employees 21 years of age with 6 months of service are eligible to participate
- Deferrals between 1-50% of earnings permitted, up to the IRS annual max ($17,500 for 2013)
- CHS matches 100% of the first 1% of employee deferral and 50% of the next 5% deferred by
the employee. Max employer match is 3.5%
- A wide variety of funds are available
- You may rollover funds from other qualified plans
- Employees are automatically enrolled at an initial 3% deferral rate, unless they choose
otherwise

*** Full time employees have an annualized EIB accrual rate of 80 hours, equal to 3.08 hours per pay
Period. Part time annualized accrual rate is 40 hours, 1.54 hours per pay period, without regard to
years of service. EIB may accrue to a maximum of 867 hours.
Extended Illness Bank
At CHS facilities, extended illness bank (EIB) hours accrue each pay period for use beginning the following pay period. Following an employee’s introductory period, EIB hours may be used in whole
shift increments after an employee has been off due to personal or immediate family illness for two consecutively scheduled shifts per occurrence. EIB usage for family illness is limited to 24 hours per occurrence.

**Holiday pay is only paid for specified shifts per current policy
